We are currently considering two qualification pathways for our Admissions Coordinator position: a Registered Nurse (RN) or a Social Worker. Only one Admissions Coordinator position will be filled, and the selected pathway will be determined based on candidate qualifications, experience, and organizational needs.

This position plays a critical role in supporting Augusta Health’s mission and advancing departmental goals through measurable performance indicators and service excellence. This position contributes to a collaborative, patient-centered environment and helps drive outcomes aligned with organizational priorities. The Admission Coordinator will work with the hospital, home health, other facilities (hospitals, ALF, SNF) and hospice team to coordinate care across the continuum. The hours are mostly daylight with occasional evenings and weekends as required to meet patient and family needs.
