Job summary

Augusta Health in Virginia's Shenandoah Valley seeks an Admissions Coordinator to coordinate care across hospital, home health, and hospice teams. The role supports referrals, discharge planning, and communication with physicians, with mostly daytime hours and occasional evenings/weekends.

Qualifications

Education: Diploma or Associate Degree in Nursing.

Responsibilities

Coordinate care across hospital, home health, hospice, and other facilities.
Communicate effectively with patients, families, hospital staff, and physicians.
Assist with hospice intake, referrals, and discharge planning.
Document in patient records and ensure timely completion of paperwork.
Educate hospital staff on hospice benefits and care processes.
Promote hospice philosophy and provide program continuity.
