Qualifications

• Bachelor of Science in Social Work required.
• BLS certification required.
• 2 years of social work experience in health care.
• 1 year of social work experience in palliative care or hospice.
• Valid driver’s license and eligibility to work in the United States.

Responsibilities

• Coordinate hospice referrals, admissions, and transitions of care.
• Consult with hospital staff regarding hospice patients.
• Communicate with attending physicians, hospice physicians, and other involved physicians.
• Educate hospital staff regarding hospice benefits.
• Complete and timely documentation in the hospice record and policy-compliant paperwork.
