The position emphasizes collaboration, quality outcomes, and seamless patient care across the continuum, with blocks of daylight hours and occasional evenings or weekends as needed.

Qualifications

• Education: Diploma or Associate Degree in Nursing.
• Licensure/Certification: Virginia State RN License.
• Experience: 2 years RN experience in clinical care.
• 1-year current RN experience in hospice.
• Driver’s License: Yes (To drive an Augusta Health Vehicle)
• Eligibility to work in the United States and meet Virginia state employment requirements.

Responsibilities

• Possesses a working knowledge of nursing principles.
• Is clinically competent.
• Communicates and collaborates with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
• Enters referral information via Intake process and coordinates care across the continuum.
• Provides care utilizing infection control measures (OSHA).
• Participates in the Hospice’s QAPI program.
