Qualifications

• Bachelor's degree in social work required; MSW preferred.
• CPR and BLS certifications are required.

Responsibilities

• Conduct comprehensive psychosocial assessments within five days of hospice election.
• Assist with advance directives, discharge planning, and family counseling.
• Provide counseling on end-of-life issues.
• Coordinate care with IDG/IDT and participate in on-call rotation.
• Make social work home visits and support patients across life span.
