Affinity Hospice Management LLC is seeking an Admission RN to plan, organize and direct hospice care in home and community settings. This role requires nursing experience and the ability to coordinate patient care with families and the interdisciplinary team.

You will perform comprehensive assessments, develop and revise care plans, administer medications, educate patients and caregivers, and act as case manager for assigned caseload, reporting to the Clinical Director.

Responsabilidades

• Completes initial and ongoing comprehensive patient and family assessments.
• Delivers professional nursing care using the nursing process.
• Evaluates patient status and revises the plan of care as needed.
• Develops care plans with goals based on nursing diagnoses and family involvement.
• Administers medications and treatments per physician orders.
• Acts as Case Manager for assigned caseload.
• Maintains accurate clinical notes and communicates with physicians and care team.
• Teaches patient and family/caregiver self-care and health instructions.
