Job summary

Optum Home & Community Care is seeking a Nurse Practitioner or Physician Assistant to perform in-home health assessments for Medicare Advantage and other plans. The role emphasizes education, gap closure, and collaboration with PCPs to improve health outcomes.

Responsibilities include reviewing medical history, vitals, and medications, and identifying urgent situations while coordinating with care teams. Flexible scheduling supports work-life balance, with visits from 8am to 8pm over seven days.

Qualifications

• Active NP certification (ANCC/AANP) required.
• Active PA certification (NCCPA) and state licensure required.
• Unrestricted licensure to practice in your state (or ability to obtain by start date).
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and conduct assessments.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plan members.
• Perform past medical history review, vitals, and physical examination.
• Coordinate care with primary care providers and educate members.
• Identify urgent or emergent situations and intervene appropriately.
• Address social determinants of health and provide referrals as needed.
