Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to conduct in-home health assessments for Medicare Advantage members. The role emphasizes education, gap closure, and collaboration with primary care providers.

Responsibilities include comprehensive home visits, documentation, and communication with PCPs. Flexible scheduling, no on-call, weekends, or holidays, and a supportive environment are highlighted.

Qualifications

• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date)
• Current and valid driver's license with access to reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments in different settings
• State scope of practice compliance and licensing requirements for NPs/PAs

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to address care gaps.
• Educate members on disease processes, medications, and adherence.
• Identify urgent situations and intervene appropriately.
