Job summary

Optum HouseCalls seeks Advanced Practice Clinicians to perform comprehensive in‑home health assessments for Medicare Advantage and other plans. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes.

You will operate in a flexible schedule within a 8am–8pm window, with no on-call, weekends, or holidays required, and a focus on safe, patient-centered care in patients' homes.

Qualifications

• Active ANCC or AANP certification for NPs in Family, Adult, Geriatric or Adult‑Gerontology Primary Care.
• For PAs: NCCPA certification and state licensure.
• Active, unrestricted licensure in practicing state with no disciplinary actions.
• Active and unrestricted driver's license and access to reliable transportation for home visits.

Responsibilities

• Conduct in‑home assessments and record full medical history, vitals, medications, and exams.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close gaps in care and coordinate care.
