Qualifications

• Active and unrestricted driver's license; reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other plans.
• Identify and close care gaps to improve patient outcomes.
• Educate patients on disease management, medications, and preventive care.
• Address social determinants of health and connect members with available resources.
• Collaborate with an interdisciplinary care team to support member health.
