Optum Home & Community Care seeks an Advanced Practice Clinician (NP/PA) to perform in‑home health assessments for Medicare Advantage and other plans. This field-based role focuses on education, gap closure, and collaboration with PCPs to improve outcomes.

The position emphasizes schedule flexibility, no on‑call, and no weekends or holidays, with support for work‑life balance while delivering preventive care at patients' homes.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
• For PAs: Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations.
• Active and unrestricted driver's license with access to reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments in seated/standing/kneeling positions.
• If required, obtain independent practice approval within set timelines after hire.

Responsabilités

• Conduct comprehensive in‑home assessments including medical history, medications, vitals, and physical exam.
• Document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to address care gaps.
• Identify urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
