Job summary

Colorado Coalition for the Homeless is seeking a Case Manager I to collaborate with case managers, supervisors, health team members, and community partners. You will develop and implement care plans, coordinate services, monitor progress, and provide advocacy and support to individuals experiencing homelessness or at risk.

The role emphasizes motivational interviewing, strengths-based practice, housing support, and trauma-informed care.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred.
• Basic Life Support Certification required within 30 days of employment.
• Must have and maintain a Colorado driver’s license within 30 days of hire.

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities.
• Maintains positive relationships with community partners and health providers.
• Works under direct supervision and collaborates within a team.
• Supports clients with wrap-around care management to ensure engagement.
• Transports clients as needed and accompanies residents to appointments.
• Assist clients in applying for and securing benefits (Social Security, Medicaid, food assistance).
