Job summary

Colorado Coalition for the Homeless is seeking a skilled Case Manager I to work with families, children, and individuals experiencing homelessness or at risk. You will develop care plans, coordinate services, monitor progress, and advocate for clients within a trauma-informed care framework.

The role focuses on outreach, referrals to mainstream services and housing, and crisis intervention to promote long-term stability for clients and their families.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, or Criminal Justice preferred.
• Basic Life Support Certification required within 30 days of employment.
• Colorado driver’s license required within 30 days of hire.

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities.
• Maintain positive relationships with community partners and health providers.
• Works under direct supervision and supports teamwork and problem-solving.
• Support clients with wrap-around care management to ensure engagement and crisis support.
• Acts as liaison between program management, client referrals, and partners.
• Respond to crises, provide de-escalation and referrals.
• Develop and implement crisis and safety plans for clients.
• Mediate conflicts and maintain positive relationships.
• Track client data and outcomes in the EHR and other systems.
