Job summary

Colorado Coalition for the Homeless is seeking a Case Manager I to collaborate with health teams and community partners to develop and implement care plans for individuals and families experiencing homelessness.

You will coordinate services, track progress, provide advocacy, and maintain accurate case documentation in the EHR while applying motivational interviewing and trauma-informed approaches to outreach and crisis intervention.

Qualifications

• Must have case management experience and ability to develop care plans.
• Experience coordinating services with community partners and health providers.
• Knowledge of trauma-informed care and recovery-focused interventions.

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities.
• Develops and implements care plans, coordinates services, and monitors progress.
• Provides advocacy and support, facilitating referrals to services.
• Maintains case documentation and data collection in the EHR.
• Delivers outreach services and crisis intervention as needed.
