Qualifications

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred.
• Basic Life Support Certification required within 30 days of employment.
• Must have and maintain a Colorado driver’s license within 30 days of hire (with exception of SSHC).

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities including substance use and behavioral health needs.
• Maintain positive relationships with community partners and health providers.
• Work under direct supervision within department parameters and in a team setting.
• Support clients with wrap-around care management to ensure consistent communication and crisis engagement.
• Facilitate referrals to external services and coordinate care with providers.
