Job summary

Colorado Coalition for the Homeless is seeking a Case Manager I in Denver to coordinate care plans, referrals, and services for individuals and families experiencing homelessness. You will work with integrated health teams to promote long-term housing stability and wellness.

The role emphasizes outreach, advocacy, and data-driven client support, including crisis planning and linkage to mainstream services. A strong commitment to trauma-informed care is essential.

Qualifications

• High School Diploma or GED required.
• Bachelor’s degree preferred in relevant fields.
• BLS Certification required within 30 days of employment.
• Colorado driver’s license required within 30 days of hire.

Responsibilities

• Manages a caseload of clients with high-service utilization and disabilities including substance use and mental health needs.
