Job summary

The Case Manager I at Colorado Coalition for the Homeless collaborates with case managers, supervisors, and partners to promote long-term housing solutions for clients experiencing homelessness. You will develop care plans, coordinate services, monitor progress, advocate, and maintain documentation in the EHR.

You will deliver outreach, facilitate referrals to housing and mainstream services, and provide crisis intervention, using motivational interviewing and trauma-informed approaches.

Qualifications

• High school diploma or GED required; bachelor's degree preferred in relevant fields.
• Experience with housing services, case management, and advocacy is valued.
• Strong communication, organization, and data documentation skills.

Responsibilities

• Manage a caseload with high-service utilization and diverse needs.
• Develop and implement individualized care plans and coordinate services.
• Monitor client progress, provide advocacy, and maintain case documentation.
• Deliver outreach, facilitate referrals to housing and mainstream services.
• Provide crisis intervention and apply motivational interviewing techniques.
