Automatic Data Processing, Inc. is seeking a Service Coordinator I - Generalist for the Sacramento Homeless Services Program. This role focuses on outreach, engagement, and housing linkage for individuals experiencing homelessness, with ongoing case management responsibilities.

Primary duties include street outreach, building rapport, assessing needs, and coordinating with local partners to provide housing solutions and retention support. The position offers full-time hours and benefits.

Qualifications

• BA in psychology or social work or 3 years related work experience with homeless population.
• Case management experience essential, including charting progress and documentation requirements.
• Ability to coordinate outreach and housing linkage with community partners.

Responsibilities

• Conduct street outreach and engagement for motel voucher and housing coordination programs.
• Assess individuals for mental illness, co-occurring substance use, and other health needs; link to services.
• Complete VI-SPDAT surveys and document results in CES or HMIS within required timeframes.
• Identify immediate needs and triage to housing, health, or resources; coordinate with partners.
• Maintain accurate files and data in agency records; document services using G.I.R.P. format.
