Step Up on Second, in Sacramento, seeks a Service Coordinator I Outreach to provide street outreach and engagement, case management, and linkage to services for unhoused individuals. The role includes coordinating with city departments and community partners, maintaining records, and supporting data reporting.

Responsibilities include assisting with enrollment in services, ensuring timely documentation, and collaborating with local outreach teams.

Qualifications

• Bachelor's degree preferred. Experience in public housing or access to public funding is required.
• Strong computer skills with Microsoft Office and intermediate MS Excel required.

Responsabilités

• Engage people experiencing homelessness to assess service needs and establish rapport.
• Carry a caseload to 25–30 cases and coordinate services accordingly.
• Evaluate immediate needs and triage to housing, health, and financial resources.
• Document and update case notes in the EHR system within 24 hours of engagement.
• Support data collection and quarterly/annual reporting.
