Job summary

The Service Coordinator I - Generalist (Outreach Specialist) is part of the Step Up Sacramento Homeless Services Program, located in Sacramento. This full-time, non-exempt position focuses on street outreach, engagement, housing linkage and ongoing case management for individuals experiencing chronic homelessness.

After enrollment, the role transitions to SCI-Care Coordinator to sustain housing retention. Responsibilities include coordinating with City Departments, documenting in HMIS/Welligent,

Qualifications

• Experience with outreach and housing-related case management is preferred.
• Knowledge of homeless services and coordinated entry processes is a plus.
• Ability to document in electronic health or program records per policy.

Responsibilities

• Conduct street outreach and engagement with clients referred to the program.
• Provide housing linkage, ongoing case management, and support services.
• Maintain program documentation and submit quarterly/annual reports.
