El Hogar Community Services, Inc. seeks a community-based professional to provide direct services to adults experiencing homelessness. You will conduct outreach, screen for eligibility, and coordinate with clients and providers to deliver recovery-oriented supports.

You will perform risk assessments, develop service plans, assist with housing navigation, and maintain accurate HMIS records, with a focus on crisis intervention and crisis response in Sacramento County.

Qualifikationen

• Minimum qualifications include HS diploma or GED and 4 years in mental health or drug/alcohol rehab with homeless populations, or 2 years direct care plus relevant training, or a BA in a related field.
• Experience in case management, crisis intervention, and coordination with community service providers is preferred.
• Ability to perform intake, risk assessments, and develop recovery-oriented plans.

• Coordinate with clients, support persons, and community providers.
• Deliver crisis intervention and transportation to appointments as needed.
