Job summary

Illumination Foundation is seeking a Medical Coordinator to deliver high quality medical care for patients experiencing homelessness in recuperative care settings. Flexible schedules include weekends and nights, with on-site responsibilities.

The role requires an LVN license in California, at least two years of relevant experience, a High School Diploma, and valid CA driver’s license. Expect a payroll range of $30.00 to $35.00 per hour plus shift differentials and a focus on compassionate,

Qualifications

• Current and non-restricted LVN license in California.
• 2 years of relevant experience.
• High School Diploma or equivalent.
• Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
• Valid CA Driver’s License.

Responsibilities

• Perform initial intake to establish an appropriate care plan based on discharge information.
• Assess health status on arrival and check vitals.
• Set weekly goals and verify goal completion for patient recovery.
• Conduct morning, afternoon, and evening checks for all clients.
• Coordinate home health visits and follow up on medical appointments.
• Provide patient education on illnesses, medications, and preventive measures.
• Arrange transportation for appointments as needed.
• Document incidents and report emergencies to supervisor.
