Job summary

Circle the City in Phoenix, AZ is seeking a Patient Navigator to join its integrated health care team. This role focuses on improving access to care, coordinating referrals, and supporting patients through enrollment and care plans.

You will engage with homeless populations, assist with registrations, and work across community resources while documenting progress and outcomes in line with organizational policies.

Qualifications

  • High school diploma required; BSW (or similar) preferred.
  • Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendors.
  • Experience in working with culturally diverse, chronically homeless, and medically needy individuals preferred.
  • Demonstrates excellent oral and written communication skills.
  • Bilingual skills – Spanish highly desirable;
  • Must demonstrate critical thinking, problem-solving, and organizational and time management skills.

Responsibilities

  • Enable non-clinical services that facilitate access to timely and appropriate care, such as referrals to transportation, case management, and education.
  • Engage in community outreach activities as directed by Integration Manager / Leadership.
  • Assist patients with completing brief applications and registration forms.
  • Assess for eligibility of DES benefits and coordinate benefits enrollment through follow-up with uninsured patients.
  • Provide education and support to patients with chronic health concerns.
  • Assist patients in understanding care plans and instructions; document activities and barriers.
