Circle the City in Phoenix, AZ is seeking a Nurse Practitioner to provide high-quality holistic health care across our medical respite centers, clinics, and mobile unit. You will work within an interdisciplinary team to deliver comprehensive care to people experiencing homelessness.

Formación

• Licensed to practice as a Nurse Practitioner in Arizona.
• Certified as a Family Nurse Practitioner.
• Minimum of three years’ experience in Family, Internal, or Emergency Medicine.
• Experience with underserved populations highly desirable.

Responsabilidades

• Assess patient health by interviewing and examining patients.
• Collaborate with physicians to develop treatment plans.
• Order and interpret diagnostic tests and labs.
• Document care and ensure continuity of care and care transitions.
