The Nurse Practitioner role involves working within an interdisciplinary team across medical respite centers, mobile units, and service sites to deliver high-quality care and support patients on their journey out of homelessness.

Qualified applicants are licensed as Nurse Practitioners in Arizona, with Family Nurse Practitioner certification

Qualifications

Licensed to practice as a Nurse Practitioner in the state of Arizona.
Certified by the state as a Family Nurse Practitioner.
Minimum of three years’ experience in Family/Internal/Emergency Medicine.
Experience with underserved populations highly desirable.
Must be at least 21 years of age.

Responsibilities

Assessing patient health by interviewing patients, performing examinations, obtaining and updating medical histories.
Collaborating with physicians to formulate treatment plans.
Ordering and interpreting diagnostic tests and lab results.
Instructing and counseling patients on regimens and wellness.
Documenting patient care and maintaining records.
Ensuring continuity of care through patient management plans and transitions.
Maintaining a safe and clean clinical environment.
Adhering to infection control policies and procedures.
Ensuring compliance with legal and regulatory requirements.
