Job summary

Circle the City in Phoenix, AZ is seeking a Nurse Practitioner to join our interdisciplinary team in providing holistic, accessible healthcare to people experiencing homelessness.

You will assess patients, develop treatment plans, coordinate with physicians, order tests, and ensure safe, continuous care across medical respite centers and mobile units. Experience serving underserved populations is highly desirable.

Qualifications

• AZ Nurse Practitioner license required.
• Family Nurse Practitioner certification required.
• Minimum 3 years in Family Medicine, Internal Medicine, or Emergency Medicine.
• Experience with underserved populations preferred.

Responsibilities

• Assess patients by interviewing, examining, obtaining and updating medical histories.
• Collaborate with physicians to plan and implement treatment—including order tests when needed.
• Order and interpret diagnostic tests (x-rays, ECGs, labs) and communicate results.
• Instruct and counsel patients on regimens for wellness and disease management.
• Document care in patient records and ensure continuity of care across settings.
• Maintain infection control and safety standards; comply with laws and regulations.
• Work as a collaborative member of the healthcare team to deliver integrated care.
