Circle the City in Phoenix, AZ is seeking a Health Navigator to support indigent, unhoused, and uninsured patients who frequently use emergency departments. You will screen, refer, and connect patients to primary care and community resources, coordinating with hospital staff and Circle the City’s Medical Respite Centers.

Qualifications include a HS diploma or GED with 2 years of direct patient care experience (MA/CM/BHT), strong communication skills, and Spanish bilingual ability preferred.

Qualifications

• HS diploma or GED required; 2 years of direct patient care experience.
• Strong communication with health professionals, patients and vendors.
• Proficiency with Microsoft Outlook, Word and Excel.
• Bilingual Spanish is preferred.

Responsabilités

• Outreach and identification of chronically unhoused patients.
• Screen patients for social, mental health, and medical needs and create action plans.
• Coordinate referrals and transition of care to Circle the City's Medical Respite Centers.
• Collaborate with shelter services, substance use programs, mental health programs and community partners.
• Assist patients to complete applications for housing, benefits, and transportation services.
