Job summary

Accra is seeking a Basic 245D Service Coordinator in Minnesota to meet with participants, set up and monitor licensed services, and explain DSP processes. You will liaise with clients, representatives, and county case managers to ensure timely service delivery.

You will stay current on Minnesota regulations, participate in committees, and support staff with knowledge of 245D services and related policies.

Qualifications

  • Four-year degree in a field related to human services, education, or health with one year of full-time direct care experience.
  • Two-year degree with two years of full-time direct care experience.
  • Diploma in human services/education/health with three years of direct care experience.
  • Minimum of 50 hours of education and training related to human services and disabilities; four years of direct care experience under supervision.

Responsibilities

  • Determine Responsible Party and DSP competency.
  • Maintain communication with client, designated representatives, and county case manager.

About This Role:

The Basic 245D Service Coordinator is responsible for meeting with, setting up, and monitoring services for participant-directed Basic 245D licensed services offered, by Accra, through the State of Minnesota. The Service Coordinator provides customer service to participants which includes, but is not limited to, completing intake meetings, ongoing monitoring of services, explaining Direct Support Staff (DSP) employment and timecard processes, determining Responsible Party and DSP competency, and maintaining communication with the client, their designated representative(s), and county case manager. The Service Coordinator serves as a resource to other Accra employees about Basic 245D Services. The Service Coordinator is expected to stay abreast of current policies and changes to rules and regulations related to Basic 245D services, and to participate in committees and meetings to enhance their knowledge.

Requirements
Must meet one of the following requirements:
  • Four- year degree in a field related to human services, education, or health and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children.
  • Two-year degree in a field related to human services, education, or health and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children.
  • Diploma in a field related to human services, education, or health from an accredited postsecondary institution and three years of full- time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children.
  • Minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications listed above.
Additional requirements:
  • Demonstrated knowledge of services needed by people with disabilities and the elderly in Minnesota.
  • Demonstrated creativity, critical thinking, and problem-solving skills.
  • Able to affect change and demonstrate positive leadership.
  • Knowledge of federal and state statutes applicable to services for people with disabilities and the elderly in Minnesota.
