How You’ll Make a Difference

• Participate in a two-week clinical orientation
• Assess, evaluate, and develop care plans for patients
• Provide education and care strategies for patients and families
• Collaborate with VNA clinicians and external providers
• Document changes and update care plans

Qualifications

• Active RN License (BSN preferred)
• 1+ year of supervised RN experience (home health or med-surg preferred)
• Excellent communication and problem-solving skills
• Valid driver’s license and access to a car

Working Conditions & Physical Demands

Work involves indoor and outdoor environments with clinical equipment, phone, and computer use. The role includes client and customer contact.
