How You’ll Make a Difference

• Participate in a two-week clinical orientation
• Assess, evaluate, and develop care plans for patients
• Provide education and care strategies for patients and families
• Collaborate with VNA clinicians and external providers
• Document changes and update care plans

Active RN License (BSN preferred)

1+ year of supervised RN experience (home health or med-surg preferred)

Excellent communication and problem-solving skills

Car and valid driver’s license

Working Conditions/Physical Demand

Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics Client and Customer contact
