• Be the Expert: Assess patients with complex health needs, develop personalized care plans, and ensure the best outcomes.
• Make a Connection: Educate and guide patients and families through their healthcare journey.
• Collaborate for Success: Work closely with VNA clinicians and external providers for seamless care.
• Document with Purpose: Record patient condition changes and update care plans accordingly.

Qualifications

• Registered Nurse (RN) with a Bachelor’s Degree in Nursing (preferred).
• 1+ year of clinical experience as an RN (homecare experience a plus).
• Strong communication skills, both verbal and written.
• Proficient with EMR systems and digital tools.
• Reliable transportation, valid driver’s license, and own car.
