Responsibilities
How You’ll Make a Difference:
  • Participate in a two-week clinical orientation
  • Assess, evaluate, and develop care plans for patients
  • Provide education and care strategies for patients and families
  • Collaborate with VNA clinicians and external providers
  • Document changes and update care plans
Qualifications
  • Active RN License (BSN preferred)
  • 1+ year of supervised RN experience (home health or med-surg preferred)
  • Excellent communication and problem-solving skills
  • Car and valid driver’s license
Working Conditions/Physical Demands

Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact
