Salary: $35.50 - $39.50 / per hour

Maxim Healthcare is hiring a Registered Nurse (RN) to join our Transition team. Our Transition Team is a specialized group of skilled nurses focused on efficiently discharging patients to home care while recruiting primary nursing staff. This team operates after we receive a patient referral for home care services and the patient is admitted to Maxim.

The Transition Team provides care for all authorized hours during a provisional period of 1-90 days. During this time, our office staff recruits and trains new caregivers, who shadow the Transition Team nurses before working independently. Once trained, these caregivers take over the patient's care, and the Transition Team moves on to new admissions.

Responsibilities:

• Utilize the nursing process to plan, implement, and evaluate patient care and services
• Initiate and revise the care plan as needed based on patient status
• Apply therapeutic, preventive, and rehabilitative nursing actions based on diagnosis and goals
• Respond to changes in patient condition and report to physicians or supervisors
• Administer medications and treatments, monitor and document effectiveness
• Review care plans, identify education, coordination, and discharge planning needs

Requirements:

• Current RN license for the practicing state
• Current Health Certificate (per facility or state regulation)
• Current PPD or Chest X-Ray
• Current BLS card
• One year RN experience preferred
