Job summary

Bloom Healthcare is seeking a Primary Care NP/PA to deliver relationship-based, in-home and community-based primary care for homebound seniors in Loveland, CO. You will work with an interdisciplinary team, manage chronic and acute conditions, and coordinate care using our data systems.

This role offers a manageable schedule, 8-12 patients daily, light call, Mon-Fri, with strong benefits starting Day 1 and a $5,000 sign-on bonus.

Qualifications

• Nurse Practitioner or Physician Assistant with a master's degree and active Colorado licensure.

Responsibilities

• Serve as the primary care provider for a defined patient panel in homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with clinical support.
• Address social determinants of health and access barriers for patients.
• Conduct specialty visits such as wound care and post- hospital transitional care.
• Complete comprehensive evaluations including medication reconciliation and health education.
• Coordinate care using Bloom’s data ecosystem to improve outcomes.
