Job summary

Bloom Healthcare is seeking a Primary Care NP/PA to deliver patient-centered, home-based primary care for seniors in Parker, CO. You’ll work autonomously with support from an interdisciplinary team and advanced data systems to improve outcomes.

The role emphasizes independence, complex primary care, and a mission-driven culture with a strong support system. Ideal candidates hold a master’s degree, CO licensure, and a passion for serving medically complex populations.

Qualifications

• Master’s degree required; NP or PA with Colorado licensure.
• Experience serving homebound seniors and medically complex populations.
• Strong communication and trust-building in home-based environments.

Responsibilities

• Provide longitudinal primary care in patients’ homes and communities.
• Diagnose and manage chronic and acute conditions with team support.
• Address social determinants of health and caregiver needs.
• Conduct wound care and post-discharge follow-ups as needed.
• Perform comprehensive evaluations including meds reconciliation and risk assessments.
• Track health outcomes using Bloom’s data ecosystem and coordinate post-discharge visits.
• Collaborate with care teams to ensure continuity and reduce readmissions.
