Job summary

Bloom Healthcare in Loveland, CO, is seeking a Nurse Practitioner or Physician Assistant to deliver primary care in home and community settings. You will see 8–12 patients daily, with autonomy and strong support from an interdisciplinary team and advanced data systems.

Responsibilities include longitudinal primary care, managing chronic and acute conditions, addressing social determinants of health, and coordinating post-discharge visits. A master’s degree and Colorado licensure are required.

Qualifications

• Master’s degree required (NP or PA).
• Active Colorado licensure as NP or PA.
• Experience in primary care with medically complex patients is a plus.

Responsibilities

• Serve as primary care provider for a defined panel in home and community settings.
• Diagnose, manage, and treat chronic and acute conditions with team support.
• Address social determinants of health and access barriers.
• Conduct wound care, transitional care, and condition-specific assessments.
• Perform comprehensive evaluations and health education for patients and caregivers.
• Coordinate care with internal/external teams to reduce readmissions.
