Job summary

Ennoble Care in Yukon/Mustang, OK seeks an experienced Advanced Provider (NP or PA) to deliver primary and palliative care to homebound patients on a part-time basis. You will work under supervision of a physician to care for a diverse caseload.

Responsibilities include performing exams, developing treatment plans, ordering tests, and coordinating with families. Strong clinical skills and ability to work independently are essential. Hospice experience preferred but not required.

Qualifications

• Board certified with valid NP/PA in Oklahoma.
• 1–2 years of primary care with geriatric patients.
• Hospice experience preferred but not required.

Responsibilities

• Provide primary and palliative care to homebound patients.
• Perform exams, obtain histories, and develop care plans.
• Coordinate with families and clinicians across platforms.
• Order diagnostics and manage medications and interventions.
• Develop impressions, diagnoses, and treatment plans.
• Maintain patient-centered, high-quality care.
