AbsoluteCARE Medical Center & Pharmacy seeks a Community-Based Clinician to deliver primary and urgent care at patients’ homes, especially after hospital discharge. The role collaborates with PCPs, care managers and ICT members to ensure seamless transitions of care.

The position involves travel across Baltimore and Prince George’s County, managing a small panel of homebound patients and addressing acute issues promptly while leveraging EMR reporting tools.

Qualifications

• This is a community-based position requiring home visits.
• Certified Registered Advanced Practice Provider in Adult Medicine with Acute Care experience.
• ACLS certification is required or preferred depending on role.
• Strong ability to manage transitions of care and coordinate with multidisciplinary teams.
• Proficiency with Electronic Medical Records and reporting capabilities.

Responsabilités

• Provide community-based medical and care coordination for recent hospital discharges.
• Manage a small panel of homebound members requiring primary care.
• Resolve acute issues in home and clinic or direct to the most effective solutions.
• Travel between Baltimore and Prince George's County to provide in-home care.
• Coordinate with Core Clinical Team to define visit goals and follow-up plans.
