Job summary

AbsoluteCARE Medical Center & Pharmacy seeks an experienced Advanced Practice Provider (NP/PA) to deliver community-based urgent and primary care. You will support transitions from hospital to home, manage chronic diseases, and address urgent needs to avoid ER visits, working with an interdisciplinary team.

You will assess patients, review medications, educate families, and document in the EMR, coordinating with home care agencies to ensure smooth continuity of care.

Qualifications

• Nurse Practitioner/Physician Assistant with 2+ years of experience.
• DEA license and unrestrictive medical license in the state of practice.
• Experience with care transitions and chronic disease management.

Responsibilities

• Provide community-based medical and care-coordination for recent hospital discharges.
• Assess patients, review and adjust medications, and perform medication reviews.
• Educate patients and families, coordinate home care, and develop care plans.
• Document in EMR and communicate with the care team to prevent readmissions.
• Support safe discharge and address barriers to care in the community.
