Job summary

Qualifications

• DEA license held.

Responsibilities

• Provide community-based medical and care-coordination services for recent hospital discharges.
• Work with transitional care manager and primary care team following discharge plans.
• Conduct patient assessments and review/adjust medications.
• Perform comprehensive medication review for adherence.
• Provide patient/member and family education.
• Coordinate home care services and develop plans of care.
• Communicate patient medical and social needs to the care team.
• Document in the Electronic Medical Record.
• Gather home environment information to inform care decisions.
