AbsoluteCARE Medical Center & Pharmacy in Columbus, OH seeks a Community Based Advanced Practice Provider (NP/PA) to deliver urgent and primary care in the member’s community. The role partners with primary care and the interdisciplinary team to ensure safe discharge transitions, manage chronic conditions, and provide preventive care.

Qualifications

• Active, unrestricted NP or PA license.
• DEA registration.
• Experience with care transitions and reducing readmissions.
• Experience with high-risk or medically complex patients.
• Familiarity with Electronic Medical Records and reporting capabilities.
• Strong computer skills, including MS Office.

Responsabilités

• Provide community-based medical and care-coordination services for recent hospital discharges.
• Collaborate with primary care and care teams to follow discharge plans.
• Conduct patient assessments and review medications.
• Coordinate home care services and develop care plans.
• Document in the EMR and communicate health needs.
• Intervene to avoid unnecessary hospitalizations and readmissions.
