AbsoluteCare in the United States is seeking a Community Based Nurse Practitioner supporting home- and community-based care within our Integrated Care Team. The CB-NP and Community MA provide hospital follow-up and discharge planning, medication reconciliation, and patient education.

The role may carry a patient panel and requires driving to member homes, performing vitals, phlebotomy, vaccines with provider authorization, and care coordination to ensure smooth transitions and prevent

Competenze

• Certified or Registered Medical Assistant in applicable state or valid NREMT-A/Paramedic with current state certification.
• 3 years MA experience required for MA candidates; EMT candidates need at least 1 year pre-hospital experience.
• High School Diploma or equivalent is required.
• Active CPR Certification is required.
• Phlebotomy skills are required.

Mansioni

• Act as the community-based medical assistant supporting primary, transitional, and acute care.
• Drive to and from the medical center and member home/community.
• Manage CB-NP schedule and follow-up appointments.
• Prepare members for in-person and telehealth exams; take vitals; conduct screenings.
• Complete medication reconciliation and document medical and social history.
