Qualifications

• Nurse Practitioner or Physician Assistant with 2+ years’ experience.
• DEA license.
• Unrestricted state medical license.
• Experience in care transitions and reducing readmissions.

Responsibilities

• Provide community-based medical and care-coordination for recent hospital discharges.
• Assess patients and review medications; conduct comprehensive med reviews.
• Coordinate with home care, family education, and plan of care.
• Document in EMR and maintain patient privacy.
• Collaborate with primary care and interdisciplinary team.
