Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations for members in their own homes, bridging gaps in care with compassionate, patient-centered assessments.

Ideal candidates are MDs or DOs with active licenses, willing to travel locally and within licensed states. You will perform visits (approximately 40 hours per week), use an iPad-based workflow, and coordinate with plan providers to deliver diagnostic and preventive services in the home.

Qualifications

• MD or DO with active license(s) in coverage area(s)
• Multi-state licenses (preferred)

Responsibilities

• Join a national network of clinicians dedicated to improving health outcomes.
• Visit millions nationwide to connect people with the right care.
• Collaborate with Signify Health colleagues in the defined role.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
• Provide Diagnostic & Preventive Services as indicated.
