Job summary

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Conduct in-home visits to assess health status and questions in a private residence.
• Review medications and medical history during visits.
• Provide diagnostic and preventive services per health plan directives.
• Travel locally and within licensed states as needed.
• Offer virtual visits when required.
• Collaborate with Signify Health care team to coordinate care.
