Job summary

Signify Health, part of CVS Health, seeks a Full Time Physician to provide in-home health evaluations, delivering personalized care at members’ homes and bridging gaps in care.

Visits last about 45 minutes with history, exam, and plan; no prescribing or lab orders. An iPad and clinical workflow are used.

Requires MD or DO with active license(s) in coverage area; multi-state licenses preferred. Some local travel within licensed states is needed; 40 hours per week.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).
• Able to cover primary locations: Berks, Bucks, Chester, Delaware, Montgomery or Philly Counties.
• Demonstrated ability to visit patients in home settings.

Responsibilities

• Bring your clinical expertise to in-home visits.
• Bridge gaps in care by connecting members with health plans and care teams.
• Work as a clinician within Signify Health network.
• Travel locally within licensed states as needed.
• Conduct virtual visits when required.
• Provide required diagnostic and preventive services as indicated.
