Responsibilities include physical exams, care plan development, order of diagnostics, and collaboration with care teams to ensure high-quality, patient-centered services.

Qualifications

• Board-certified NP with MA license.
• Strong clinical skills to assess, diagnose, and treat.
• Ability to work independently with elderly and palliative patients.
• Excellent communication with patients and care teams.

Responsibilities

• Provide primary and palliative care to homebound patients in Boston, MA region.
• Perform physical exams and develop care plans.
• Order diagnostics and prescribe treatments as needed.
• Coordinate with physicians and staff across platforms.
• Attend medical staff meetings and participate in care planning.
