Optum Home & Community Care is hiring a HouseCalls Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other plans. You will educate patients, coordinate with primary care providers, and address social determinants of health in a field-based role.

Flexible scheduling, no on-call, weekends, or holidays; 24–40 hours weekly. Competitive total compensation with base pay plus bonus and comprehensive benefits.

Formación

• For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For PAs: Current NCCPA certification and state licensure
• Unrestricted licensure in practicing state (or ability to obtain by start date)
• Reliable transportation for home visits
• Ability to navigate varied home environments during assessments

Responsabilidades

• Conduct in-home assessments for Medicare Advantage and other plans
• Perform history review, vitals, exam, and point-of-care testing as appropriate
• Document diagnoses for care management and treatment planning
• Coordinate with PCPs to close care gaps
• Identify urgent situations and intervene as needed
• Educate members on diseases, medications and adherence
• Address social determinants of health and provide referrals
