Ennoble Care is seeking an experienced Advanced Practice Provider (NP or PA) to provide primary and palliative care to homebound patients in the Philadelphia region on a full-time basis. You will apply medical techniques and principles under physician supervision and care for patients with chronic diseases in their homes.

Responsibilities include exams, care planning, ordering tests, prescribing medications, and coordinating with families to ensure compassionate, patient-centered care.

Qualificações

• Board Certified, with a valid NP or PA certification in PA.
• Strong clinical skills to accurately assess, diagnose, and treat.
• Positive demeanor and strong interpersonal skills.
• Ability to problem solve and operate independently.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.

Responsabilidades

• Provide high-quality primary and palliative care to homebound patients in the Philadelphia, PA region.
• Perform physical examinations and develop care plans based on patient histories.
• Communicate effectively with office staff and clinical teams across platforms.
• Deliver patient-centered care to a panel of homebound patients.
• Develop impressions, diagnoses, and treatment plans.
• Foster trust and consult with patients, families, and caregivers during visits.
• Order diagnostic testing as medically necessary.
• Prescribe medications and necessary interventions.
