Job summary

Ennoble Care is a mobile primary and palliative care provider offering services across multiple states. We seek an experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver high-quality palliative care remotely, applying medical techniques under physician supervision and caring for a diverse patient population with chronic conditions.

You will assess health, create comprehensive care plans, and collaborate with the patient’s care team to ensure goal-concordant

Qualifications

• Board-certified NP or PA in NJ, PA, GA, VA, OK, KS.
• 2–3 years of palliative, chronic care management or primary care experience with geriatric patients.
• Ability to work remotely and provide patient care virtually.

Responsibilities

• Provide high-quality palliative care remotely to patients in NJ, PA, GA, VA, OK, KS.
• Perform physical examinations and develop care plans.
• Communicate effectively with office staff and clinicians across platforms.
• Collaborate with care teams to develop holistic plans of care.
